Basic Initial Consultation
This consultation is designed to evaluate the behavior and describe what a behavior change plan would entail. This is a good start if you are curious about the behavior and how it may be changed.
What is included:
✔ Pre-consultation questionnaire
✔ 1 hour virtual consultation for behavior history collection, behavior observations, behavior goal setting, and discussion of recommended behavior change plan
✔ 1 hour in-home session to start implementing goals
✔ 15 minute phone follow-up

Additional 1 hour in-home sessions as needed $175
* Additional travel fee may be required outside of service area

Behavior Change Package #1
This consultation package is designed to evaluate the behavior and to begin behavior change sessions. Package #1 includes the initial consultation and 3 behavior change sessions to focus on 1-2 behaviors. Additional behavior change sessions can be added after a package is completed.
What is included:
✔ Pre-consultation questionnaire
✔ 1 (in-home or virtual) initial session for behavior history collection, behavior observations, and goal setting
✔ 3 (1 hour, bi-weekly) behavior change sessions (in-person and/or virtual)
✔ 3 (15 minute) mid-session check-ins via phone or zoom
✔ Behavior change plan reports with videos and protocols after each session
✔ Video review with feedback
✔ Progress tracking for success

This consultation package is designed to evaluate the behavior and to begin behavior change sessions. Package #2 includes the initial consultation and 6 behavior change sessions to focus on more than two behaviors or more complex behaviors. Additional behavior change sessions can be added after a package is completed.

✔ 1 (in-home or virtual) initial session for behavior history collection, behavior observations, and goal setting
✔ 6 (1 hour, bi-weekly) behavior change sessions (in-person and/or virtual)
